For my clients who are navigating the complex landscape of long-term care options, I stress the importance of planning ahead. Clients are best able to ensure that their loved ones are cared for through careful long-term planning. I would like to share some tips to assist you and your loved ones in creating the most effective long-term care plan.
- Assessing Care Needs: The first step in nursing home planning is a thorough assessment of the individual’s needs. This step requires evaluating one’s physical health, daily living requirements, and cognitive abilities. Many of my clients consult physicians or geriatric care managers in order to determine the level of care needed. It is also crucial that the individual’s preferences are taken into account when developing this plan.
- Financial Planning: It is no secret that the care provided by nursing homes can be expensive, so diligent financial preparation and planning is crucial. Families should explore their loved one’s financial resources, including savings, investments, and insurance coverage. It is important to note that in North Carolina, Medicaid is a primary payer for nursing home care for individuals who qualify.
- Legal and Estate Planning: Establishing advance directives, such as a well written Healthcare Power of Attorney and Living Will, guarantees that the individual’s wishes regarding medical care and finances are honored if they are rendered incapacitated. Because of this, it is ideal for families to engage in estate planning, including Wills, Trusts, and asset protection strategies, while the individual is mentally and physically well. Consulting with an elder law attorney before an individual needs long-term care is the best way to ensure their wishes are respected.
- Medicaid Planning: For those who anticipate qualifying for Medicaid assistance to cover nursing home costs, Medicaid planning is essential. This involves structuring assets and income in a way that complies with Medicaid eligibility rules while preserving assets for the individual or their spouse. It is important to note that this planning must be done far in advance in order to qualify due to the Medicaid five-year look-back rule.
- Emotional Support: Moving into an assisted living facility is a huge transition for the individual and their family. It is important for the individual’s family to provide support during this transitional period. I would encourage the individual’s loved ones to not only visit, but to encourage them to engage with their new community.
- Continuing Care Options: Some clients could benefit from alternative care options, such as assisted living facilities or home care services. Exploring these options alongside nursing home care can help families make the best choice for their loved one’s needs and preferences. Additionally, it is helpful to consider specialized care, such as memory care for individuals with dementia, to ensure that the individual receives tailored care that meets their unique needs.
- Regular Review and Monitoring: Nursing home planning is an ongoing process that requires regular review and monitoring. As the individual’s care needs evolve, adjustments may need to be made to their care plan, financial arrangements, or legal documents. Staying informed and proactive ensures that the individual receives appropriate care and support as they age. Regular communication with the nursing home staff, healthcare providers, and legal advisors can help identify any issues or changes that require attention and ensure that the individual’s care plan remains up-to-date and effective.
